RE: Vaccination Clinic
 
Great News, Two H1N1 vaccination clinics have been scheduled by Maine General in Augusta.

The first scheduled clinic will be on Tuesday, Dec. 22, at the Elks Lodge. This clinic will be open from 10am to 6pm. It is free of charge. The clinic is open to everyone. First time H1N1 vaccinations for children and adults will be available as well as booster vaccinations for the children 9 and younger.
 

The second H1N1 clinic offered by Maine General will be for the school children of Augusta. This clinic will be held on Friday, Jan. 8, at Cony. The school department will bus the children and staff over to Cony during the school day. Both booster vaccinations and first time vaccinations will be available for those who have yet to receive the H1N1 vaccination.

If you would like your child to have a vaccination at the Augusta School Clinic you will need to fill out a permission form. If you did not receive a permission form with this letter, you can download one at www.augustaschools.org, request one from your school office or click on Permission Form. Please return permission forms as soon as possible.
